Job Description:
Essential FunctionsThe Clinical Practice Manager serves as the
liaison between the Post-Acute facility and the regional
leadership. The Clinical Practice Manager, under the direction of
the Vice President of Operations (VPO), is responsible for
operational performance of the facilities in his/her assigned
region. The Clinical Practice Manager provides operational support
to their PODS, coordinates client service activities and performs
special assignments and other related work as required* Develop and
implement business development and retention strategies with
primary care and specialty providers.* Develop and implement
business development and retention strategies with managed care
organizations.* Develop and implement business development and
retention strategies with LTAC, SNF, and Sub-Acute facilities* Lead
onsite meetings with providers and client facilities on a quarterly
basisCore Competencies and AttributesResults Orientation:*

attitude. Hands on.* Demonstrates that he/she has the analytical
problem-solving skills necessary to help a healthcare system
accomplish its mission.* Will be able to point to examples of
having delivered outstanding results in quality and performance
improvement initiatives.* A good business sense; able to link
quality care and patient safety to improved hospital financial
performance.Strategic Orientation:* Ideally the candidate will not
only have created the vision within his/her current function, but
also raised broader issues, influencing overall organizational
strategy, thereby contributing to the larger planning process and
helping shape the future of the entire organization.Collegiality
and Teamwork:* Evidence of an ability to create a culture of
collegiality and respect for both the community and academic
providers contribution to success as evidenced by a history of
removing barriers, providing resources, and skill at facilitating
the subordination of individual needs in order to achieve a common
goal.* Group-oriented, collaborative, and exhibits a positive
attitude and appropriate humility. Must be a problem solver, team
player, and be self-motivated and driven.* Must be focused on the
development of people including strong skills in mentoring,
coaching, building effective teams, selecting provider management
talent, and building and growing skill sets.* Leads by example and
through influence.Required Qualifications & Experience* 3 to 5
years of work experience in a healthcare related field* BS or
equivalent, in a healthcare related field* Knowledge of basic math
and the ability to perform math functions* Ability to analyze and
create complex financial schedules* Ability to work well under
pressure of meeting deadlines and with difficult people* Ability to
establish priorities and be flexible enough to adapt to changing
ones* Ability to multi-task* Ability to communicate effectively -
orally, in writing, in person and on the telephone - with all
categories of people within the company and potential affiliates*
Knowledge of spelling, grammar, punctuation and English usage
sufficient to write professional presentations* Knowledge of
medical terminology is a plus* Knowledge of computer sufficient to
produce reports accurately and efficiently* Proficiency with
Microsoft Office Suite, including Excel, Word* Ability to work
independently and in an organized manner* Ability to establish
goals and demonstrated ability to complete projects by deadlines*
Ability to establish priorities and be flexible enough to adapt to
changing ones* Ability to work well under pressure of meeting
deadlines and with difficult people* Ability to travel up to 50%*
Ability to attend company and professional functions after normal
work hours
