JOB TITLE Case Manager II Virginia - Intellectual & Developmental Disabilities

REPORTS TO Divisional Senior Vice President

FSLA STATUS Non-exempt


Coordinating services for individuals with developmental disabilities (DD) within a caseload. This includes the administrative maintenance of all case records and any corresponding case management duties assigned to them.


Education: Bachelor's degree in behavioral health related area required.

Licensure: Must possess or obtain highest level of licensure appropriate to degree. Valid drivers license.

Certification: Must possess or obtain highest level of certification appropriate to degree.

Experience: Experience in mental health and/or intellectual disabilities preferred.

Knowledge/Skills: Verbal/written communication skills.

Ability to maintain composure in difficult situations.

Ability to safely operate motor vehicle.


Computer, fax, copier, motor vehicle and any other equipment required to perform the functions of the position.


The process of service coordination requires that the case manager possess the knowledge, skills, and abilities necessary to perform the following activities:

Assessing - evaluating and determining all of the service needs of the individual and/or family. This includes completing a VIDES assessment (to determine qualification for waiver services) and participation and coordination of Supports Intensity Scale (SIS) assessments.

Planning - formulating written Individual Service Plans. These Individual Service Plans must be person-centered and should address the following:

1) strengths and weaknesses of the individual

2) service needs that clearly identify specific goals and objectives. These goals and objectives should clearly as possible identify specific target dates for the attainment of these services or the resolution of needs.

3) responsibility for the coordination and the delivery of identified services.

4) development and discussion of employment goals

Linking - Advocating for the individual in contacting and actively facilitating constructive relationships with service resource agencies, programs, individuals, etc. This includes linking individuals to REACH crisis services and managed care organizations.

Referral - Referral of individual to other community agencies, medical providers, Regional Support Teams, etc. to improve overall health of individual

Monitoring - An ongoing process of reviewing and updating Individual Service Plans, insuring that services are intact, and that they are adequately meeting the needs of the individuals. Includes up-to-date and accurate record-keeping (progress notes, quarterly reviews and other required documentation) in WAMS, CMCSB's electronic health record, etc.


Provide basic counseling, which includes supportive counseling, family assessments, and crisis intervention.

Coordinate appropriate referrals to vocational training programs such as sheltered employment, supported employment programs, Department of Aging and Rehabilitation, etc.

Coordinate all residential placements within the community.

Coordinate all necessary institutional placements. This includes the completion of any applications, pre-admission screenings, etc. and participation in any treatment planning conferences, and discharge planning.

Conducting intakes for all individuals in need of DD services.

Promotion of community awareness and family education regarding DD services and available resources, providing individuals with access to various providers (their choice of providers).

Participation in required agency staffings, and various interdisciplinary meetings with other agencies or community resources.

Participation in a continuing education program designed to improve professional knowledge of concepts associated with developmental disabilities, supported counseling, and emergency services.

Respond to crisis situations that may arise involving individuals with developmental disabilities.

Provide written documentation to complete all necessary chart work in accordance with policy on admissions treatment and discharges in a timely manner.

Maintain client confidentiality according to Frontier Health policies
