  • This position works within the following Crisis Programming: Behavioral Health Urgent Care Center (BHUCC) and Knox Crisis Stabilization Unit (CSU).
  • Serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for the client population in which they have been assigned.
  • The float nurse is responsible for the direct oversight of psychiatric care to the assigned clinics or programs based off need.
  • The float nurse may be given pre-assigned coverage based off annual leave or given same day coverage if a nurse is out unexpectedly.
  • The float nurse must be adept at multi-tasking. Will be required to provide direct medical care to clients in a crisis setting, a residential setting, or in a case management role, frequently working with SPMI and clients with co-occurring issues.Summary of position:
    • Assists training and covering for nursing staff for BHUCC and Knox CSU.
    • Works with Crisis Services RN Coordinator to ensure all shifts are covered at all times which may require working overnight, weekend, and holiday shifts.
    • Provides direct care to clients as needed and assists with crisis interventions and medical emergencies.
    • Coordinates with all team members regarding client discharge planning and serves as a liaison with other agencies as indicated.
    • This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
    • This position requires utilizing a personal dependable vehicle to conduct Center business.
    • Maintaining a dependable vehicle and certified driver status is a condition of employment.
    • Regular attendance is an essential job function.
    • Mandatory to remain awake and alert during shift.TYPICAL WORKING CONDITIONS/ENVIORMENT
      • Position is based within Crisis Services which provides services 24/7.
      • The role of the Crisis Services Floater Nurse is to ensure that coverage is provided 24/7 despite any weather conditions that may arise.
      • This position requires working different shifts as needed.Daily Staff Duties:
        • Attends treatment team as available.
        • Responds to all flags, emails, and voicemails within 1-2 business days.
        • Returns calls within 15 minutes.
        • Assists with administering medications and providing staff coverage as needed.
        • Clearly communicates to team members throughout shift regarding client behaviors, needs, and any other relevant information.
        • Works in a multi-disciplinary team to meet the needs of the clients served.
        • Demonstrates a consideration and concern for fellow workers and their jobs and promotes harmonious relationships and attitudes.
        • Willingness to accept additional assignments and / or changes in assignment and/or work duties.
        • Promotes an environment in which the customs and spiritual beliefs of the individual are respected.
        • Communicates directly and approaches conflict with a problem-solving approach.Crisis Unit:
          • Performs the initial admission nursing assessment and completes required admission paperwork including nursing assessment, nursing note and nursing admission orders.
          • Update medication tabs.
          • Update allergy lists.
          • Complete paper MAR.
          • Obtain urine drug screen, BAL and pregnancy tests upon admission.
          • Follows all established protocols and guidelines for admission prior to admitting the client to the unit.
          • Makes the determination of the appropriate admission protocols needed for each client (ex.-agitation protocol, diabetic protocol, HTN protocol).
          • Obtains admission orders from the on-call prescriber in a timely manner.
          • Completes charting for each client during every shift prior to the end of shift.
          • Completes transition paperwork and discusses medication and aftercare instructions with client and/or family. Provides copy of paperwork for clients.
          • Counts controlled substances and documents at every shift change. Notify supervisor immediately of any discrepancies.
          • Obtains and charts vital signs on every shift.
          • Monitor for significant changes in client's condition/behavior every shift and make referrals to obtain new orders as needed.
          • Administer scheduled, routine, and PRN medication in a timely manner.
          • Obtains blood sugars as ordered.
          • Triages potential referrals with Mobile Crisis and Walk in Center.
          • Coordinates arrival times for potential clients with Mobile Crisis, Walk in Center and Transporters
          • Complete C-SSRS on each client
          • Attend to guest and/or client needs during medical emergencies (ie: seizures, falls, etc)
          • Coordinate with Emergency Departments and EMS
          • Obtain Prior Authorizations
          • Answer phone calls from recently transitioned guests regarding medication issues.
          • Weekly medication counts of routine medications.
          • Update Admission/Discharge log each time a client is admitted or transitioned from unit.
          • Complete follow-up calls on weekends in accordance with program requirements.Residential/Inpatient:
            • Perform the initial medical assessment of individual within 30 minutes of arrival.
            • Makes a determination of the appropriate detox protocol upon admission.
            • Completes all chart documentation within required time frames and adheres to licensure agency, state and CARF Standards.
            • Monitors client's vital signs and record them in EMR.
            • Will administer medications accurately and safely following the "5 Patient Rights".
            • Complete a nursing note 3 x per 8-hour shift, 4 x per 12-hour shift and PRN.
            • Review with Client ASAM Placement criteria and obtain Clients signature.
            • Will be able to complete nursing assessment within 2 hours.
            • Work with multi-disciplinary team to meet client needs and maintain clinical standards of the program.
            • Ensure that the correct Standing Orders/Protocols are noted for clients being admitted.
            • Complete all work assignment within shift worked.
            • Assist clients to obtain prescribed medications, follow up with pharmacy to ensure that the prescription is available.
            • Attend all mandatory staff meetings.
            • Must be punctual and maintain a good attendance record.
            • Perform other duties as assigned by the Nursing Manager and Team Leader.PROFESSIONAL DEVELOPMENT:
              • Complete and maintain Relias Training Modules when required.
              • Maintain CPR and HWC certifications up to date.
              • Maintain and possess a current nursing licensure. Schedule:
                • Work non-structured hours with understanding that schedule is subject to change to meet the needs of Crisis Services Programming.
                • Will provide coverage for other Nursing Staff.
                • Will be on-call for program needs.
                • Daily in-person attendance is essential.
                • Must give six hours' notice if not able to work their scheduled shift (unless a documented emergency arises).Travel:
                  • Travel is expected for the float nurse as their assignments may differ on a day-by-day basis.
                  • The float nurse will be expected to travel between programs at times and may be required to travel for trainings or meetings.
                  • There is no overnight travel.
                  • Must have or be able to obtain an F-endorsement on driver license within first 2 weeks of employment.
                  • This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.Equipment/Technology:
                    • Basic computer skills are required for email, timekeeping, and documentation in the electronic medical record.
                    • Must be familiar with basic Microsoft Office Skills.
                    • Must be able to use a fax machine and phone system.QUALIFICATIONS - Crisis Services Floater NurseEducation:
                      • Must have LPN or RN degree with a current/valid Tennessee license with course work or experience in the areas of cultural diversity, human development, primary care, etiology, and treatment of mental illness, alcohol and drug use, physical and sexual abuse, suicide, and intellectual developmental delays.
                      • Must demonstrate a working knowledge of supervisory, administrative, and clinical skills.Experience / Knowledge:
                        • LPN certification or Associate/Bachelor degree from an accredited College/University.
                        • Experience in psychiatric nursing and acute care preferred.
                        • Must have the ability to present professionally, work within a team format, and have the mental ability to exercise sound, clinical judgment under pressure.
                        • Must have the ability to communicate effectively (both oral and written) and possess good time management and organizational skills.
                        • Basic computers skills required; experience with electronic medical record (EMR) preferred.
                        • Experience with interdisciplinary collaboration and care coordination preferred.Physical:
                          • Exposure to biological hazards.
                          • Hearing of normal/soft tones and close eye work.
                          • Valid driver's license- "F" Endorsement.
                          • Lifting up to 50 lbs. Pushing/pulling up to 150 lbs.
                          • Frequent sitting, standing, walking, bending, stooping, and reaching.
                          • CPR and First Aid certification required (training provided).
