education to discharge planning and follow-up, you’ll be the
connector that makes the patient journey seamless. What You’ll Do:
Support and guide care teams to deliver outstanding outcomes;
Coordinate patient care plans and discharge planning. Promote best
practices, safety, and efficiency across inpatient and outpatient
settings. Serve as a resource and set the standard for professional

hours/week | Day Shift Qualifications - Licensure: Current RN
(Registered Nurse) license in state of Missouri. - Required
Education: Graduate from an accredited school of nursing -
Preferred Education: Bachelor of Science in Nursing (BSN) -
Required Certifications: Active BLS (Basic Life Support)
certification - Preferred Certifications: ACLS Advanced Cardiac
