role for an Physical Therapist Assistant Therapist at Outbound

Responsibilities: 1. Implement the Plan of Care prescribed by the
supervising physical therapist, asjusting as needed based on
patient response. 2. Employ exercise, manual therapy, therapeutic
activities, neuromuscular re-education and or modalities. 3.
Monitor existing Plan of Care as it relates to established
functional and objective goals. 4. Regularly consult with and
discuss patient status and progress with supervising PT. 5.
Instruct patient in the use of DME supplies. 6. Develop and educate
patient/family regarding HEP’s. 7. Assist in the collection of
objective measures and functional outcome measures. 8. Maintain
complete and accurate documentation per visit regarding treatment
tolerance, progression, and subjective reports. 9. Participate in
case conferences, clinical reviews 10. Communicates with
agencies/team members and supervisors as appropriate. 11. Maintain
patient rights and confidentiality. 12. Participate in new employee
mentorship program. 13. Participate in performance during first 90
days and once a year thereafter. 14. Maintain accurate and
up-to-date personnel requirements. 15. Participate company
inservices and quality improvement initiatives Outbound Physical

Associate's Degree Licensure to practice as an Physical Threrapist
Assistant in the state of Missouri Excellent interpersonal and
