Residential LPN - Part Time The Helen Ross McNabb Center (www.mcnabbcenter.org) is a premier not-for-profit provider of behavioral health services. The Center provides quality care and serves all ages in a continuum of prevention, early intervention, social, co-occurring, addiction, outpatient and crisis services. The Center is seeking motivated staff members that possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels and economic circumstances with respect and dignity. Excellent verbal, written and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as, maintaining proper documentation. Clinical staff members must be able to take and pass, without major modifications, a course in Handle with Care. This program requires hands-on, physical components to ensure the safety of the clients and staff. Clinical staff members must possess reliable transportation, ability to acquire an F endorsement on driver's license, and transport clients. HRMC is an Equal Opportunity Employer (EOE). HRMC is currently seeking a qualified and enthusiastic professional for the following position: Residential LPN - Part Time. JOB REQUIREMENTS Education/Knowledge: LPN with current Tennessee license and experience in psychiatric nursing preferred. LPN certification as Psychiatric/Mental Health Nurse preferred. Should have sound knowledge of psychiatric medications, diagnoses, treatment modalities and interventions. Ability to use computer. Experience: Experience in psychiatric nursing preferred. Experience in working with special populations including individuals with Severe and Persistent Mental Illness, Co-Occurring Disorders, and Dual Diagnosis Disorders preferred. Computer experience is helpful. Experience working in a crisis setting preferred. Physical: Exposure to biological hazards. Hearing of normal and soft tones. Close eye work. Frequent exposure to unpleasant odors. Lifting up to 50 lbs. Pushing/pulling up to 250 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching. JOB SUMMARY Serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for client population. LPN must be adept at multi-tasking. Provides direct medical care to clients, including clients in varying levels of crisis, frequently working with SPMI and client's with co-occurring issues. Assists with crisis interventions and medical emergencies, obtains vital signs each shift or more frequently as indicated. Administers intramuscular injections, communicate with PCPs during intake and discharge phases, and liaison with other agencies as indicated. LPN is responsible for triaging referrals, completing intake/assessments on all clients admitted during their shift, obtaining urine drug screens and pregnancy tests on admissions, obtaining admission orders, dispensing routine and PRN meds, taking off orders from prescriber and updating MAR, completing electronic and paper charting in a timely manner and participating in tx team meetings. Additionally, LPN may take calls from recently discharged clients requesting bridging rx, follow-up calls on weekends on discharged clients, obtain prior authorizations and coordinate with pharmacies. LPN collaborates with Mobile Crisis assisting with BHUCC client's being referred to higher level of care, completing S-BARs. Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking. Schedule requires rotating weekends. Regular attendance is an essential job function. This job description does not constitute a written or implied contract of employment. JOB TITLE: BHUCC LPN 1. Daily Staff Duties a. Performs the initial medical assessment and completes required admission paperwork including nursing assessment, nursing note and nursing admission orders. b. Update med tabs. c. Update allergy lists. d. Complete paper MAR. e. Obtain urine drug screen and pregnancy tests upon admission. f. Follows all established protocols for admission prior to admitting the patient to the unit. g. Makes the determination of the appropriate admission protocols needed for each patient (ex.-agitation protocol, diabetic protocol). h. Obtains admission orders from the on-call prescriber in a timely manner. i. Completes charting for each patient during every shift prior to the end of shift. j. Completes discharge paperwork and discusses medication and aftercare instructions with patient and/or family. Provides copy of paperwork for patients. k. Counts narcotics and documents at every shift change. Notify supervisor immediately of any discrepancies. l. Obtains and charts vital signs on every shift. m. Monitor for significant changes in client condition/behavior every shift and make referrals to obtain new orders as needed. n. Administer scheduled, routine, and PRN medication in a timely manner. o. Obtains blood sugars as ordered. p. Triages potential referrals with clinician. q. Coordinates arrival times for potential BHUCC clients r. Complete C-SSRS on each client each shift s. Attend to client needs during medical emergencies (ie: siezures, falls, etc) t. Coordinate with Emergency Departments and EMS u. Obtain Prior Authorizations v. Answer phone calls from recently discharged clients regarding medication issues w. Weekly med counts of routine meds x. Update Admission/Discharge log each time patient is admitted or discharged from unit. y. Complete follow-up calls on weekends in accordance to program requirements. 2. Team Building a. Must be punctual and maintain a good attendance record. b. Participates in daily treatment team meetings. c. Participate in shift reports. d. Coordinate discharges with discharge planner and treatment team. e. Maintains a positive and professional working relationship with all staff members and supervisors. f. Communicates and approaches conflict in a problem solving manner with staff and supervisors. g. Accepts additional assignments and/or changes in assignments to meet the needs of Crisis Services. h. Assists when necessary to obtain vital signs on clients being referred to a higher level of care. i. Promotes an environment in which cultural diversity and customs, values and beliefs of individuals are respected.
