education and training but your eligibility begins with you meeting
the initial requirements. A Family Based Service Provider:
May be married, single or divorced
May or may not have other children
Can own your home or rent
Must be at least 18 to adopt or 21 to become a foster parent
Must reside in the state where the client comes from
Must be able to meet the financial and emotional needs of your

Must be a resident of the U.S.
How do I become a Family Based Services Provider?
The first step is to complete our information request form on
our website or to contact the person noted as the Coordinator of
Family Based Services (you will also find this information on the
website). After completing this process, a Support Solutions team
member will contact you and will work with you to complete the
application and approval process. Upon approval to provide
services, Support Solutions will assign team members who will work
to make the match for you and your new family member(s). Where
possible, meet and greet sessions will be held to give you and the
family an opportunity to connect with the person you will make a
member of your family.
Family-Based Providers are sub-contractors and Support Solutions
is noted for ethical management practices, competitive

includes nursing services, behavior supports, case management
