JOB SUMMARY
At Houston Methodist, the Triage Specialist position is responsible
for high level clinical triaging and Telehealth patient care,
utilizing a clinical evidence-based protocol assessment. The
position provides an assessment of patient needs and determining
the appropriate care disposition, based on protocols, collaboration
with clinic staff members, provider preferences, and organizational
standard operating procedures. The Triage Specialist position
utilizes technology and electronic medical records to provide
information to patients by phone. This position conducts telephone
triage calls which involves critical thinking and analysis through
clinical decision making, effective interpersonal skills and strong
oral/written communication.
PRIMARY JOB RESPONSIBILITIES

PEOPLE - 25%

SERVICE - 30%
- Leads the patient and family-centered care standards, as
outlined in the Houston Methodist Professional Nursing Practice
document. Interprets and describes patient assessments, identify
nursing interventions. (EF)
- Anticipates and recognize/resolves early declines in patient
condition and emergent situations. Recognizes an emergency or high
priority situation and take appropriate and immediate action.
Prioritizes care based on the patient's needs, abilities and
preferences. Advocates on patient/family's behalf to identify and
resolve clinical and ethical concerns. Develops appropriate
resources and supports unit practices and guidelines. (EF)
- Develops and implements effective patient teaching strategies
based on learning needs; uses appropriate resources, incorporating
planning for care after discharge/disposition. Consistently
evaluates the patient's comprehension and adapts teaching methods
accordingly. (EF)
- Drives department activities to improve department score for
patient satisfaction, through peer-to-peer accountability to
service standards. (EF)
QUALITY/SAFETY - 20%
- Models the standards related to regulatory requirements and
professional practice as outlined in the Houston Methodist
Professional Nursing Practice document. Role models highly
developed assessment, clinical documentation, leadership and
analytical skills; integrates and contributes to professional
practice standards, policies, procedures, protocols and leads
improvements in patient safety-related processes at department or
service line level. (EF)
- Analyzes information, problems, situations, practices, and
procedures to recognize alternatives and provide solutions.

- Leads initiatives to improve quality and safety scores, through
peer-to-peer accountability, reporting near misses and identifying
solutions by collaborating with the interprofessional team. Role
models situational awareness, using teachable moments to improve
safety. (EF)
FINANCE - 10%
- Models independent time management effectively and
prioritization of daily tasks, minimizing incidental overtime.
Actively participates in the stewardship and efficient utilization
of resources and needs including capital and operational budget
needs. (EF)
- Implements department strategies to achieve financial target
and mentors others to do the same through timely documentation of
care, optimizing efficiency, minimizing incidental overtime, and
other areas according to department specifications. (EF)
GROWTH/INNOVATION - 15%
- Promotes individual professional growth and development by
meeting requirements for mandatory/continuing education and skills
competency; supports department-based goals which contribute to the
success of the organization; serves as preceptor, mentor and
resource to less experienced staff. Completes and updates the
individual development plan (IDP) on an on-going basis. (EF)
- Champions new care pathways or service line implementations to
improve care to the community. Identifies trends for improvement
opportunities or practice changes and supports initiatives.
Researches the change and presents options to appropriate clinical
leadership. (EF)

EDUCATION REQUIREMENTS
- Bachelor of Science degree or higher from an accredited School
of Nursing
EXPERIENCE REQUIREMENTS
- Three years' direct patient care experience, preferably in a
large, complex, integrated delivery system; such as acute or
critical care, Emergency department, triage or ambulatory
environment
- Experience with a variety of specialties & sub-specialties
preferred
- Experience in entering clinical notes in an EMR system, EPIC
preferred
CERTIFICATIONS, LICENSES AND REGISTRATIONS REQUIRED
- Registered Nurse with current Texas licensure
- Board Certified in Ambulatory Nursing Care through ANCC
preferredKNOWLEDGE, SKILLS AND ABILITIES REQUIRED
- Ability to interpret and describe patient assessments and
identify nursing interventions. Demonstrates ability to make quick
decisions, based on sound base of critical thinking
skills/guidelines by protocols. Skill in recognizing an emergency
or high priority situation and taking appropriate and immediate
action.
- Demonstrates the skills and competencies necessary to safely
perform the assigned job, determined through on-going skills,
competency assessments, and performance evaluations
- Sufficient proficiency in speaking, reading, and writing the
English language necessary to perform the essential functions of
this job, especially with regard to activities impacting patient or
employee safety or security
- Ability to effectively communicate with patients, physicians,
family members and co-workers in a manner consistent with a
customer service focus and application of positive language
principles
- Ability to work with Electronic Medical/Health Records and
document electronically
- Demonstrates ability to make quick decisions, based on sound
base of critical thinking skills/guidelines by protocols
- Familiarity with Nursing regulations, institutional standards
of care and risk management
- Advanced clinical knowledge of medical diagnoses, procedures,
