Overview:
Registered Nurse - Quality Care ManagerFull Time, 80 Hours Per Pay
Period, Day ShiftFort Loudoun Medical Center is equipped with a
team of more than 200 doctors in morethan 29 specialty areas. Our
87,000-square-foot hospital features advanced technology, including
state-of-the-art Magnetic Resonance Imaging (MRI), Computerized
Tomography (CT), Ultrasound, Diagnostic X-Rays and Women's Imaging
Services, as well as Nuclear Medicine technology unique to
oursurrounding counties. The physicians, staff and volunteers of

excellentcare to every patient, every time.Position Summary:
Responsible for the planning, organizing, management and evaluation
of operations of quality, care management, infection prevention,
regulatory compliance and medical staff services of assigned
facilities. Responsible for the coordination of care for all
patients through the continuum of care in assigned facilities.
Fluent in quality improvement principles and Care Management
requirements. This position manages all HR activities including
daily staffing for discharge planning activities for responsible
departments. This position reports to the District Director and the

Responsibilities:
In collaboration with District Director, develops department
budgets that reflect effective management of resources to provide
services.In collaboration with District Director, facility
leadership and medical staff, assists in the development of
strategic initiatives for the department and facility.Works with
facility leadership to identify and monitor key processes and
patient outcomes. Facilitates effective communication of results to
physicians and other key stakeholders.Identifies opportunities for
improvement, implements corrective action plans to resolve issues
and evaluate results for desired outcomes.Manages Care Managers and
social work functions to ensure the coordination of care for
patients so they move seamlessly through the continuum of
care.Stays abreast of regulatory and licensing (Joint Commission,
CMS, Leapfrog, State etc) requirements and ensures processes are in
place to achieve desired outcomes.Oversees peer review activities
at assigned facility.Collaborates with clinical and medical staff
to ensure financial and length of stay goals are achieved.Oversees
continuity of strong Care Management Model to ensure compliance
with established processes. Serves as a coach, mentor and role
model for staff.Oversees all activities of medical staff office,
infection prevention and regulatory specialist ensuring compliance
with regulatory/licensing requirements.Ensures compliance of core
measures and evidenced based care on a concurrent basis.Works with
leadership of all Covenant entities to ensure staffing needs are
met in all facilities.Proactively seeks opportunities to reduce
waste and redundancy in the utilization of resources.Participates
in performance improvement teams to improve processes related to
the coordination of care.Actively participates as on the Quality,
Peer Review and Credentials Committees.Effectively communicates
with medical staff to improve utilization of resources and
compliance with evidenced based care.Collaborates with nursing and
other caregivers to ensure processes are in place to prevent
hospital acquired conditions and hospital acquired
infections.Ensures multidisciplinary rounding process occurs with
the appropriate people in attendance.Provides current reliable data
to all stakeholders on an ongoing basis to ensure clinical and
financial goals are met.Attend meetings, prepare reports and make
presentations as required to physicians and hospital groups
including senior leadership.Local travel required.Follows policies,
procedures, and safety standards. Completes required education
assignments annually. Works toward achieving goals and objectives,
and participates in quality improvement initiatives as
requested.Performs other duties as assigned.
Qualifications:
Minimum Education: None specified; however, must be sufficient to
meet the standards for achievement of the below indicated license
and/or certification as required by the issuing authority.Minimum
Experience: Five (5) years' experience with at least three (3)
years in a hospital setting; Experience and/or education in quality
improvement, HIM, risk management or related field
preferred.Licensure Requirements: Must have and maintain Tennessee
State RN license. CCM, ACM or CPHQ certification required or must
be working toward and obtain within eighteen (18) months of
employment in position. Employee must have a valid Tennessee
driver's license Class D and state mandated minimum insurance
coverage. Driving record must meet Covenant Health minimum
standards at the date of hire and throughout employment tenure.
