You will provide personalized physical therapy and rehabilitation services to patients in their homes, ensuring the highest standard of care. Assess patient's physical abilities and needs to develop customized treatment plans. Implement therapeutic interventions aimed at improving patient outcomes. Document patient progress and adjust treatments as needed. Educate patients and families on therapy techniques, health maintenance, and preventive care. Coordinate with healthcare professionals to ensure comprehensive patient care. Proven ability to perform physical therapy assessments and treatments Strong interpersonal and communication skills Ability to work independently and manage multiple patient cases Experience with electronic medical record (EMR) systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Physical Therapy license in the state of practice CPR certified A valid driver’s license and reliable transportation, as this role includes travel to patient homes
